The role covers geriatric patients in the home health setting, with a schedule of 40 hours weekly and 8-hour daytime shifts. Must have active Ohio PT license and at least 1 year of home health experience.

Oasis and HCHB EMR experience are preferred. Mileage reimbursement is provided, and locals traveling at travel rates are welcome. On-call every 3rd–4th weekend, with a standard 60-mile territory.
