Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel assignment in Salem, Arkansas. The role is a 13-week contract with 40 hours per week and 8-hour shifts, travel between patient homes included.

You will conduct PT assessments, develop plans of care, and provide interventions for strength, endurance, pain management, and fall prevention, while coordinating with the care team and adhering to Medicare/OASIS-E guidelines.

Competenze

• Active, unrestricted Physical Therapist (PT) license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience (within the last 2 years).
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).
• Reliable transportation with valid auto insurance and ability to travel between patient homes.

Home Health Physical Therapist Contract Details:

• $2,040 - $2,225 per week*
• 31-week contract
• Paid by the hour - not by the visit
• 5-6 visits per day (45-90 minutes each, based on complexity)
• Travel time factored into productivity expectations
