Qualifications

Responsibilities

• Perform plan of care interventions including medication administration and treatments.
• Monitor patient condition and communicate updates to care team and family.
• Coordinate care across disciplines including transfers and discharges.
• Educate patient and family appropriate to age and condition.
• Advocate for patient safety and wellbeing.
