Qualifications

• Minimum of one year acute care experience in hospital setting.
• Appropriate certifications as required for specific position.
• Degree from an accredited school.
• Abide by code of conduct and compliance policies and expectations.
• Ability to communicate clearly in English.

Responsibilities

• Assist healthcare professionals in delivering patient care
• Monitor, record, and communicate patient condition to care team
• Coordinate patient care across disciplines including transfer, discharge, referral, and psychosocial support needs
• Evaluate learning needs of patient and/or family and provide education
• Advocate for the physical and emotional well-being of the patient
