Job summary

Qualifications

• Minimum of one year acute care experience in a hospital setting.

Responsibilities

• Assist healthcare professionals in delivering patient care.
• Monitor, record, and communicate patient condition to care team, physician, patient, and family.
• Collaborate across disciplines to coordinate patient care including transfers, discharge, referrals, and psychosocial support.
