Job summary

HBCI is hiring an Interventionist to provide intensive, trauma‑informed crisis intervention and stabilization for youth and families in their homes and communities.

The role involves delivering evidence‑based interventions, developing treatment plans, and coordinating care with collateral providers. Travel to families’ homes is required, with on‑call rotation duties and related responsibilities.

Qualifications

• MSW or related degree with licensure or eligible licensing required.
• Experience delivering trauma-informed mental health services.
• Experience working with children and youth in crisis situations.

Responsibilities

• Provide intensive crisis intervention and stabilization for 2–3 families at a time.
• Conduct up to daily meetings with each child/youth and family.
• Deliver crisis intervention and stabilization services.
• Conduct outreach to clients and families.
• Screen and assess for social determinants of health, mental health, trauma, risk, and substances.
• Assess clients and develop Treatment Plans.
• Coordinate treatment plans with other providers.
• Maintain complete case management records.
• Participate in on‑call rotation and coverage as needed.
• Support additional projects and duties as assigned by the Supervisor.
• Engage hard-to-reach populations and families affected by trauma and poverty.
