Job summary

Nexus Family Healing in St. Cloud, MN, seeks a dedicated Social Worker to join our on-site team. You will help eligible clients and caregivers access medical, social, and educational services, and provide therapeutic interventions to children and families.

Qualifications

• Bachelor's degree in Social Work required.
• LSW license required.
• Valid MN driver's license and auto insurance required.
• Ability to pass background screening per DHS rules.

Responsibilities

• Coordinates and implements treatment needs and best interests of service recipients.
• Completes systemic assessment of client and family.
• Provides documentation within established timelines.
• Delivers skill development and therapeutic services to families.
• Creates trauma-informed environment and crisis intervention as needed.
• Advocates for funding and resources at local, state, and national levels.
• Maintains confidentiality and manages caseload with accurate records.
• Participates in supervision and team meetings.
