Qualifications

• MD, Board Certified or Board Eligible provider licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty
• Proficient in electronic medical records (eClinicalWorks preferred)
• Proficient in Microsoft Office, including Outlook
• Experience with homeless or justice-involved populations preferred
• Bilingual Spanish speaker preferred

Responsibilities

• Provide comprehensive primary care to adults with lived experience of homelessness in community-based settings (housing, shelters, drop-in centers, Street Medicine)
• Deliver trauma-informed, evidence-based care to medically complex populations
• Collaborate with psychiatry, behavioral health clinicians, social workers, case managers, and outreach staff to coordinate care
• Maintain accurate clinical documentation with protected administrative time
• Manage a balanced patient caseload focused on quality outcomes
• Engage in weekly protected educational time and accredited Grand Rounds
• Contribute to program development and continuous quality improvement initiatives
• Uphold equity, social justice, and culturally responsive care
