Job summary

Lutheran Services Florida (LSF) is seeking a Case Manager Intensive in Florida to provide trauma-informed, intensive case management and supportive services to vulnerable clients. The role emphasizes crisis intervention, safety planning, and coordinating culturally appropriate services within our community programs.

The position requires a Bachelor's degree (Social Work or related field preferred) and provides opportunities to impact families and individuals while collaborating with a team

Qualifications

• Bachelor's degree required in a related field.
• Strong English communication and documentation skills.
• Experience in trauma-informed case management is ideal.

Responsibilities

• Conduct weekly in-person trauma support with clients focusing on crisis intervention, safety planning and family preservation.
• Provide client-centered intensive case management for vulnerable clients with special circumstances.
• Conduct comprehensive intakes for eligible participants and develop goal-oriented self-sufficiency plans.
• Maintain timely communication with supervisor regarding client safety, stability, or well-being.
• Submit detailed documentation in line with LSF policies and procedures.
• Utilize trauma-focused interventions for ongoing services and social work best practices.
• Coordinate referrals and follow up to ensure services are linguistically and culturally appropriate.
• Supervise and train program interns and volunteers as needed.
