Qualifications

• Strong writing and communication skills required.
• Exceptional organizational skills and attention to detail.
• NYS Driver's license with a clean MVR and reliable transportation.
• Lived experience as a parent or caregiver navigating child-serving systems.

Responsibilities

• Engage families to begin partnership and connection to services.
• Conduct intake and needs assessment for family and child.
• Write treatment plans based on assessments and service components.
• Assist families in carrying out tasks outlined in treatment plans.
• Collaborate with care team and attend meetings as needed.
• Complete all required documentation and participate in supervision.
• Facilitate family peer groups if designated.
