Job summary

LifeBridge Community Services in Bridgeport, CT, is seeking a Behavioral Health Clinician (LMSW, LPCa, LMFTa) to provide trauma‑informed mental health services. You will assess clients, develop treatment plans, and lead individual, family, and group sessions within a collaborative team.

Qualifications

• Must hold CT state license (LMSW, LPCa, or LMFTa).
• Experience navigating an Electronic Health Record (EHR).
• Proficient with Office 365.
• Valid driver’s license.

Responsibilities

• Conduct client assessments and establish treatment goals.
• Develop and implement treatment plans collaboratively with client.
• Plan and facilitate individual, family, and group sessions.
• Align with a client-centered, 'unconditional positive regard' treatment approach.
• Evaluate and document client's progress and the effectiveness of therapy interventions.
• Maintain accurate, clear, and complete documentation of all facets of a client's treatment in the EHR.
• Coordinate client care with other treatment team members to ensure effective care delivery.
• Deliver coordinated services that help clients access community support when needed.
• Participate in professional development training and supervision.
• Meet agency patient access goals and metrics.
• Demonstrate strong oral and written communication and collaboration skills.
• Exhibit excellent time-management and professionalism.
