Qualifications

Responsibilities

Transporter# TransporterTuolumne Me-Wuk Tribal Council - Tuolumne, CA 95379## OverviewSalary Range$20.00 - $24.50 Hourly## Description**Duties and Responsibilities:** (specific areas of responsibility including, but not limited to)* Relates to and has knowledge of people in the community and facilitates communication between community members and health care providers.* Builds trust and confidence of clients and health care providers.* Act as client advocate to facilitate services to clients and assist in overcoming barriers to acquisition of health care.* Maintains the visitation schedule with all eligible participants of the service area and constant follow-up schedule, including clinic appointments.* Act as a case finder, identifying all eligible participants who reside in the target area.* Maintain confidentiality and conduct essential to Tribal Government operations to build confidence between staff and community.* Transports in the service area of Tuolumne County and other health care agencies per Tribal Government protocols.* Maintains consistent, accurate communication and submit required paperwork and reports to the Elders Activities Coordinator.* Attends workshops and training sessions as needed.* Organizes regular assignments to maximize outcomes and to utilize work time efficiently.* Other duties as assigned.## Qualifications**Qualifications:*** High school diploma or GED.* Must possess a valid California Driver’s License and a clean driving record.* Must be insurable by TMTC* Must have the ability to communicate effectively orally and in writing; and the ability to follow oral and written instructions. Along with sufficient written and verbal communication skills to be able to interact with patients, medical professionals, vendors, and agencies in a professional manner, developing rapport and enhancing business relationships.* Must have the ability to work in an independent and self-directed manner and assume individual initiative.* Must be willing and can accept supervision and to follow directions.* Must possess skill in using computers and other office equipment; in gathering and summarizing information; preparing reports; and in planning, organizing, prioritizing, and executing complicated and continuing transportation assignments.* Must have the ability to maintain confidentiality and handle sensitive matters with discretion and tact. Must have a general knowledge of and the ability to maintain HIPAA privacy requirements.* Must sign a confidentiality statement.* Must have the ability to work in a fast-paced and sometimes stressful environment.* Must live within and have knowledge of service area.* Must possess the ability to operate a vehicle safely in a variety of situations; to evaluate if passengers are physically able to be transported; to recognize need for passenger assistance and to be physically able to assist passengers when needed.* Position requires current CPR certificate and First Aide training.
