Job summary

Senior Care Partners PACE is seeking a detail-oriented Transportation Scheduler to coordinate safe, reliable rides for participants at our PACE Center in Michigan. You will work with participants, caregivers, healthcare staff, and transportation vendors to ensure timely access to medical appointments and PACE services.

Qualifications

  • High school diploma or equivalent required.
  • 1–2 years of scheduling or dispatch experience preferred.
  • Experience working with older adults, frail elderly, or long-term care populations preferred.
  • Strong organizational, time management, and multitasking skills.
  • Excellent verbal and written communication skills.
  • Ability to solve problems, prioritize tasks, and make sound decisions independently.
  • Proficiency with computers, scheduling systems, and data entry.

Responsibilities

  • Schedule and coordinate transportation for participant appointments and PACE services.
  • Communicate daily with clinic schedulers, referral coordinators, transportation vendors, participants, and caregivers.
  • Notify participants of transportation pick-up times and appointment details.
  • Monitor and adjust transportation schedules to accommodate changing participant needs.
  • Collaborate with the interdisciplinary care team (IDT) to support participant-centered care.
  • Participate in transportation assessments and care planning.
  • Maintain accurate documentation in participants' medical records.
  • Build and maintain positive working relationships with transportation providers and community partners.
  • Complete required training and perform other duties as assigned.

TheProgram ofAll-InclusiveCare for theElderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.

PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Senior Care Partners PACE is looking for a detail-oriented, compassionate Transportation Scheduler to coordinate safe, reliable, and efficient transportation for our participants. In this role, you'll work closely with participants, caregivers, healthcare staff, and transportation vendors to ensure every ride supports timely access to medical appointments and PACE services.

  • Must be fully vaccinated or willing to become vaccinated against:
  • Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
  • Must meet CMS competency standards prior to working independently
  • Must be willing to work beyond normal hours
  • Comfortable working in varied home environments and all weather conditions
