Job summary

HCA Healthcare's Methodist Hospital in San Antonio, TX is seeking a Transplant Social Worker (LMSW) to support inpatient and outpatient care with focus on post-acute planning for transplant patients, including lung transplant cases.

You will collaborate with RN case managers, physicians, and families to assess psychosocial needs, coordinate discharge planning, and provide emotional support during challenging transitions, with a Monday–Friday 8am–5pm schedule.

Qualifications

• Licensed to practice social work in Texas (LMSW).
• Master's degree in Social Work.
• Minimum of 2 years of social work experience; transplant experience preferred.

Responsibilities

• Collaborate with care team to identify and address patient needs.
• Assess barriers to discharge including financial, emotional, and psychosocial factors.
• Maintain knowledge of Medicare, Medicaid, and payer regulations.
• Develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings with patients, families, and care teams.
• Provide psychosocial support during end-of-life and crisis situations.
