Job summary

Qualifications

• Bachelor degree in Nursing required.
• 2+ years acute clinical experience required.
• Current active RN license in good standing in state of residence or multi-state license.
• Basic Life Support (BLS) certification required.

Responsibilities

• Coordinate care for transplant candidates and recipients across inpatient and outpatient settings.
• Educate patients and families about transplantation processes, donor options, and post-transplant care.
• Collaborate with multidisciplinary teams, coordinate evaluations, and manage documentation.
• Monitor patient progress, assist with immunosuppressive therapy, and coordinate discharge planning.
• Maintain listing status, communicate with physicians, payers, and transplant teams.
