Job summary

Lehigh Valley Health Network is seeking a transplant-focused healthcare professional to join the team as a transplant coordinator. You will facilitate patient care, coordinate transplant processes, and maintain critical waiting lists across multiple departments.

The role requires clinical experience, familiarity with CMS and UNOS regulations, and strong communication with patients, staff, and physicians. A CDA/CMA certification and BLS are preferred, with competitive benefits.

Qualifications

• High School Diploma or GED required.
• 3 years experience in a medical setting.
• Knowledge of computer operation and data entry for patient information retrieval.
• Knowledge of medical terminology and insurance processes.
• AHA Basic Life Support (BLS) CPR certification required upon hire.

Responsibilities

• Provides care to designated patients following hospital policies and procedures.
• Coordinates care for pre-transplant, post-transplant, living donor, and surgical patients.
• Reviews medical history and patient suitability; coordinates scheduling of tests and reviews results.
• Coordinates and/or participates in education of transplant and/or surgical patients.
• Coordinates organ transplant with Organ Procurement Organization, transplant candidate, physicians, and staff.
• Develops and maintains data management systems to meet requirements.
• Monitors patient labs in Histotrac and related medical records.
• Maintains an up-to-date transplant waiting list with nursing staff coordination.
• Maintains contact with transplant candidates to ensure adherence to care plans.
• Monitors and audits patient charts for CMS and UNOS compliance.
• Reviews referral information from nephrologists, dialysis units, endocrinologists, and others.

Summary

Facilitates effective clinic organization and patient care by serving as a communication link between the patients, staff, physicians, hospital departments, or other outside health care entities.

Job Duties

• Provides care to and related activities for designated patients while following hospital and department-specific policies and procedures.
• Coordinates care for pre-transplant, post-transplant, living donor, and/or surgical patients.
• Reviews medical history and suitability of patients; coordinates scheduling of appropriate medical testing and reviews test results.
• Coordinates and/or participates in the education of transplant and/or surgical patients.
• Coordinates organ transplant with Organ Procurement Organization, transplant candidate, physicians, and hospital staff.
• Develops and maintains systems for data management to meet requirements.
• Monitors patient lab results in OTTR, transplant's legal medical record.
• Maintains an up-to-date transplant waiting list by coordinating patient's activities with the nursing staff.
• Maintains contact with transplant candidates to ensure compliance with a care plan.
• Monitors and audits patients' charts to ensure that they are compliant with both CMS and UNOS regulations and requirements.
• Monitors patients' cPRA results in Histotrac to assure their transplant readiness and appropriateness of donors
• Reviews referral information from nephrologists, dialysis units, endocrinologists, or other referral sources.

Minimum Qualifications

• High School Diploma/GED
• 3 years experience in a medical setting.
• Knowledge of the operation of a personal computer and data entry for retrieval of patient information.
• Knowledge and working use of medical terminology and insurance.
• American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
• CMA - Certified Medical Assistant - AAMA - State of Pennsylvania Upon Hire or
• NCMA - National Certified Medical Assistant - NCTT - State of Pennsylvania Upon Hire or
• RMA - Registered Medical Assistant AMT - State of Pennsylvania Upon Hire or

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
