Qualifications

• Registered nurse with unrestricted license.

Responsibilities

• Respond to ADT alerts in real-time and enroll patients.
• Collaborate with hospital staff for safe transition plans.
• Conduct home visits within 2 days post-discharge.

Skills

Registered nursing license (unrestricted)

Experience in hospital-based care coordination

Strong clinical assessment and critical thinking skills

Ability to perform in-home visits

Excellent communication and patient education skills

Proficiency with electronic health records

Reliable transportation
