Job summary

Cooper University Health Care in New Jersey is seeking a part-time Case Manager to formulate discharge plans with patients, families and the care team based on a needs assessment. You will coordinate referrals to home care agencies, skilled nursing and rehabilitation centers, and community programs, and work with insurers on the authorization process.

Ideal candidates have 3–5 years of health care experience and a Master’s in Social Work or a BSN/RN with NJ licensure; ACM and CCMC certifications

Qualifications

• 3–5 years health care experience preferred.
• Requires MSW or BSW, or an RN with BSN preferred; licensure required.
• Current NJ SW License with LCSW preferred, or NJ RN License; ACM/CCMC preferred.

Responsibilities

• Formulates discharge plans with patient and care team based on needs assessment.
• Coordinates referrals to home care, skilled nursing, and community programs.
• Coordinates care authorization with insurers and Utilization Management.
• Performs psychosocial assessments and crisis intervention.

Short Description

Formulates the discharge plan with patient, families and the care team based upon a needs assessment. Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs. Coordinates care authorization process with insurers Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status

Provides psychosocial assessments, crisis intervention, resources referrals,to facilitate discharge plans, and/or adjustment to illness, andcomplex discharge planning for patients and their families.

Formulates the discharge plan with patient, families and the care team based upon a needs assessment.

Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community based programs.

Coordinates care authorization process with insurers

Experience Required

3 - 5 years health care experience preferred

Education Requirements

Requires a Master Degree in Social Work (MSW)/ or BSW or a licensed RN, BSN preferred

License/Certification Requirements

Current NJ SW License, LCSW Preferred.
