Job summary

National HealthCare Corporation (NHC) seeks a Transitions Care Coordinator to support the Middle Tennessee area. You will facilitate seamless transitions from facilities to home care and drive market growth by developing accounts and educating the medical community on our services.

Responsibilities include assessing post-acute needs, coordinating care, ensuring physician orders, and documenting Face to Face interactions.

Qualifications

• Current BSW, MSW, LPN or RN license in the state of service.
• One year of home health experience or hospital case management experience.
• Experience in business development, outreach, or healthcare-related industry.
• Reliable transportation with a valid driver's license and auto insurance.

Responsibilities

• Navigate post-acute care needs after hospital discharge.
• Assess, plan, implement, coordinate, monitor and evaluate care options to ensure safe transitions to home health.
• Ensure physician has an order for the home health plan of care and complete Face to Face documentation.
• Develop strategic plans to grow accounts and increase market share through referrals.
• Represent NHC HomeCare professionally to the healthcare community and care teams.

Transitions Care Coordinator

NHC HomeCare is looking for a Transitions Care Coordinator to join our team serving our Middle Tennessee area - Metro Nashville Market. The Transitions Care Coordinator serves to facilitate a seamless transition for patients transitioning from a facility to the Home care environment along with executing the sales strategy to increase company market share through account development and educating the medical community on our services.

Preferred Qualifications

• Current BSW, MSW, LPN or RN license in the state of service
• Must have one-year home health experience or one year of hospital case management experience.
• Successful previous experience in business development, outreach, or healthcare related industry
• Reliable means of transportation and must have current driver’s license and auto insurance

1- 2 yrs Experience in Home Health and / or healthcare business development

Position Highlights

• After patient has selected NHC HomeCare as his or her health care provider, the Transitions Care Coordinator will assist patients in the process of navigating their post-acute care needs.
• Assess, plan, implement, coordinate, monitor and evaluate options and services with a primary goal of providing a safe transition from a facility to home for home health.
• The Transitions Care Coordinator is responsible for ensuring the patient has a physician and obtains an order from that physician to oversee the home health plan of care.
• Face to Face documentation must also be obtained and communicated to appropriate agency.
• The Transitions Care Coordinator will develop strategic plans to successfully manage and grow accounts and market share.
• Grow market share by obtaining referrals and increased participation in the NHC program
• Communicate effectively with all members of the health care delivery team.
• Represents NHC Homecare in a positive and professional manner, projecting and reinforcing company objectives and philosophy.
