Job summary

Central Ohio Primary Care is seeking a Transition of Care Nurse (LPN) in Columbus, OH. The role supports planning, facilitation, and advocacy across the care continuum within COPC’s Transition of Care program.

The position is full time with standard weekday hours (8:00am–4:00pm) and requires travel to partner hospitals within COPC’s service area. Strong clinical and case management skills are essential to coordinate care and improve patient outcomes.

Qualifications

• Current Ohio Nursing License is required.
• Active American Heart Association BLS Certification required.
• LPN with at least 2 years of practice experience.
• Valid Ohio driver’s license for travel within COPC’s service area.

Responsibilities

• Be a change agent for innovative approaches to healthcare and advocate for patient-centric care.
• Assist patients through the healthcare system by advocating for them and navigating care.
• Complete assessments, identify gaps, set goals, facilitate interventions and create action plans.
• Increase patient adherence to treatment plans.
• Coordinate with community resources to meet psychosocial, financial and functional needs.
• Address care gaps in collaboration with providers, hospital RNs, and social workers.
