Molina Healthcare is seeking a Care Transition Coordinator in Seattle, WA to support transition activities from hospital to home or other settings. You will coordinate care, collaborate with discharge planners, hospitalists, and providers, and help reduce readmissions by ensuring timely follow-up and appropriate services.
The role requires at least 2 years in healthcare, knowledge of care transitions, strong communication, and ability to work independently with limited supervision.
Provides support for care transition activities. Facilitates transitional care processes and coordination for member discharge from hospital admission to all other settings. Strives to ensure that best possible services are available to members at time of hospital discharge, and focuses on goal to reduce member readmissions. Contributes to overarching strategy to provide quality and cost-effective member care.

Pay Range: $26.41 - $59.21 / HOURLY
