Job summary

Specialized Education Services, Inc. (SESI) is seeking a Transition Coordinator to support students aged 14 and up as they move from high school to post-secondary opportunities.

You’ll collaborate with special education staff, families, and community partners to address learning and life skills needs, implement SESI's evidence-based models, and promote student independence and success through individualized planning.

Qualifications

Responsibilities

  • Diagnose and address learning, social, and emotional skill gaps.
  • Collaborate with school teams and families to deliver guidance and SESI’s models.
  • Inspire and empower students through SESI’s instructional approaches.
  • Provide transition support from high school to post-secondary.
  • Maintain IEPs, BIPs, incident reports, and high-risk evaluation forms.
  • Track student growth toward academic, social, and emotional goals.
  • Supervise students during school transitions.

Responsibilities

What you can expect to do.

Uncover the light and potential in each student by diagnosing and addressing learning, social, and emotional skill gaps. Collaborate with school teams and families to offer personalized guidance and deliver SESI’s own evidence-based learning and instructional models, as well as positive behavioral interventions and support framework. You’ll inspire, empower, and transform with our innovative learning approach, unique instructional environments, smaller class sizes, and the support services students need to shine.

The Transition Coordinator will be responsible for providing academic, emotional, and physical services for students who require additional support to thrive in school. They will collaborate with special education teachers, social workers, and administrators to help students age 14 and up prepare for the transition from high school to successful post-secondary. The Transition Coordinator must be willing and able to work with students on job and life skills, community-based instruction, college and career readiness. Any experience with job corps programs, post-secondary academic institutions, or Serve-Safe or Pro-Start training (culinary arts) is a plus. The Transition Coordinator must be willing to spend time making connections in the community that will promote school and benefit students' post-secondary success. Every child deserves to reach their full potential and we’re looking to hire an exceptional transition coordinator who will play an essential role in fueling student success.

  • Work with a diverse group of students from varying developmental, cultural, ethnic, racial, and socioeconomic backgrounds.
  • Use SESI’s instructional models, strategies, and curricula to teach knowledge and skills, nurture confidence and character, and meet the needs of all students.
  • Develop and maintain individualized skill-appropriate lesson plans.
  • Implement behavior management and intervention strategies, including de‑escalation techniques, crisis management, and physical restraint when necessary to maintain the safety of students and staff.
  • Maintain student documents such as individualized education plans (IEPs), behavior intervention plans (BIPs), incident reports, and high‑risk evaluation forms.
  • Drive, record, and track student growth and progress toward academic, social, and emotional goals.
  • Supervise and monitor students, including to and from locations within the school.
