Qualifications

• Ability to read, write, type, and speak English proficiently.
• Educational prerequisites include PT program degree and licensure.
• CPR certification is required.

Responsibilities

• Interpret prescriptions, perform initial evaluation, and establish a treatment plan with functional/work goals.
• Progress treatment and goals as the patient improves.
• Communicate with physicians in a timely manner regarding progress and discharge.
• Discharge management and patient follow-up.
• Supervise rehabilitation aides and PTAs as applicable.
• Document all treatment, evaluations, progress notes, and related forms.
• Develop and maintain professional relationships with referral sources.
