Health First seeks a Transfer Access Registered Nurse Case Manager to coordinate direct admits, transfusions and chemotherapy within the IDN. You will ensure timely transfers, assess acuity, and communicate with physicians and facilities to prevent EMTALA violations.

Qualifications

Responsibilities

  • Coordinate direct admits, blood transfusions and chemotherapy across the IDN
  • Manage transportation (ground and air) for transfers
  • Organize transfers between Health First facilities and other health systems
  • Complete orders for transfusions, direct admits and transfers
  • Create bed requests for all direct admits, transfusions and transfers
  • Use acuity algorithms for emergent events and triage transfer requests
  • Identify appropriate levels of care for placement and safe transport
  • Assist with surgical scheduling
  • Communicate clinical updates to physicians effectively
  • Monitor bed availability and placement; coordinate with staff and other systems
  • Conduct conference calls with admitting, sending, referring and specialty providers
  • Maintain accurate documentation of transfers, phone calls, transports and acuities

POSITION SUMMARY

The Transfer Access Registered Nurse RN Case Manager provides clinical expertise, medical communication and quality assessment for the timely facilitation and coordination of all direct admits, blood transfusions and chemotherapy across the Health First Integrated Delivery Network (IDN). The Transfer RN Case Manager will facilitate and coordinate patients transferring in or out of Health First in accordance with state and federal regulations to prevent EMTALA violations. The Transfer Access RN Case Manager will be highly effective in critical, clinical fact gathering and analysis to identify appropriate level of care, timely acceptance and specialty care needed for transport. The Transfer RN Case Manager employs critical thinking skills to expedite admissions and transfers, demonstrating Health First mission, vision, values and standards in all interactions with referring facilities, physicians, employees and customers.

The Transfer Access RN Case Manager will be highly effective in critical, clinical fact gathering and analysis to identify appropriate level of care, timely acceptance and specialty care needed for transport. The Transfer RN Case Manager employs critical thinking skills to expedite admissions and transfers, demonstrating Health First mission, vision, values and standards in all interactions with referring facilities, physicians, employees and customers.

Primary Accountabilities
  • Coordinates direct admits, blood transfusions and chemotherapy across the IDN
  • Manages transportation (ground and air) for transfers across the IDN
  • Organizes transfers between Health First facilities and other health care systems
  • Completes electronic or written orders for transfusions, direct admits and transfers
  • Creates accounts/bed requests for all direct admits, transfusions and transfers
  • Uses complex acuity algorithms for time-sensitive emergent medical events (Trauma, STEMI, Clot Alert or Code Rupture), triaging, processing transfer requests, identifying the appropriate receiving campus, and conferencing physicians for specialist acceptance
  • Identifies appropriate levels of care for placement and safe transport via ground or air
  • Assists with surgical scheduling
  • Utilizes knowledge and understanding to triage critical and non-critical patient, effectively communicating clinical updates to physicians
  • Monitors and assists with bed availability and placement, effectively communicating with PFA's, nursing staff and other health care systems
  • Harnesses conference calls effectively with admitting, sending, referring and specialty providers or APRN's
  • Maintains accurate and thorough documentation of transfers, transfer phone calls, transports and acuities, gathers and analyzes information from current and past medical records
Work Experience
MINIMUM QUALIFICATIONS
  • Education: Associate's degree in Nursing (ASN), or Nursing Diploma.
  • Work Experience: Three (3) years if clinical experience.
  • Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
  • Certification: None
  • Skills/Knowledge/Abilities:
  • Demonstrated leadership skills
  • Strong analytical, data management and computer skills
  • Excellent interpersonal and negotiation skills
  • Ability to communicate effectively with customers of varied levels of understanding
  • Current working knowledge of care transitions; transfer and receipt of medical records
  • Understanding of pre and post-acute care venues and post-acute community resources
  • Ability to prioritize multiple tasks and demands
  • Ability to work independently and exercise sound judgment in interactions with physicians, payers, patients and families
  • Excellent customer service skills with ability to promote safe and cost-efficient improvements
  • Excellent writing and presentation skills.
Preffered Qualifications
  • Education: Bachelor's degree in Nursing (BSN).
  • Work Experience: Previous experience in Case Management.
  • Certification: Case Manager Certification (CCM or ACM).
  • Skills/Knowledge/Abilities: Knowledge of utilization management, and managed care reimbursement.
Physical Requirements
  • Majority of time involves sitting or standing; occasional walking, bending, and stooping.
  • Long periods of computer time or at workstation.
  • Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
  • May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
  • Communicating with others to exchange information.
  • Visual acuity and hand-eye coordination to perform tasks.
  • Workspace may vary from open to confined.
  • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
