Job summary

Pharmko seeks a registered dietitian to join the Home Nutrition Support team. The RD will assess and manage nutritional needs for patients on PN and EN, coordinating with prescribers and pharmacists to optimize care plans.

The role includes patient education, troubleshooting, and representing Pharmko’s nutrition services to referral sources. Ideal candidates have RDN credentials, CNSC, NY licensure if applicable, and at least 2 years of clinical experience, with home infusion or nutrition

Qualifications

• RDN credential required.
• CNSC.
• Current licensure in New York State (if applicable).
• Minimum of 2 years clinical experience; home infusion or nutrition support preferred.
• Strong knowledge of ASPEN guidelines and evidence-based nutrition therapy.
• Excellent communication, clinical reasoning, and patient interaction skills.
• Ability to work independently and manage a flexible caseload.

Responsibilities

• Conduct thorough nutrition assessments for patients receiving home PN and EN.
• Review labs, weight trends, intake/output, and clinical status to recommend therapy adjustments.
• Collaborate with Pharmko pharmacists and prescribers to implement personalized nutrition regimens.
• Educate patients and caregivers on home nutrition therapy, including formula, pump usage, and monitoring.
• Troubleshoot tolerance and pump issues, and provide timely follow-up.
• Maintain accurate and timely documentation of assessments and interventions.
• Represent Pharmko’s Nutrition Program during outreach efforts to physicians, case managers, and discharge planners.
• Support the business development team with clinical insight and referral support.
