Veterans Health Administration, Department of Veterans Affairs, is seeking an Advanced Practice Nurse (APN) Certified Nurse Practitioner to provide expert care in Thoracic Surgery and Pulmonary Transplant Services. The NP will lead evaluation, diagnosis, education, and medication management across surgical and non-surgical pathways.

• Independently manages a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Provides education to patients and their families.

Responsabilidades

• The Thoracic Surgery NP, in collaboration with the interdisciplinary team, manages the care of patients in the Thoracic Surgery and Pulmonary Transplant Specialties.
• Provides comprehensive assessment, diagnosis, education and medication management for patients in both surgical and non-surgical pathways.
• Inpatient and outpatient care may be included.
• May complete virtual/Econsults in collaboration with Thoracic Surgery/Pulmonary Transplant staff.
• Works collaboratively across surgery, medicine, nursing, and other professional service lines.
• Practices to the full extent of education, training, and certification.
• Has prescriptive authority for a panel of patients or program as indicated by the position.
• Engages in leadership and contributes to population health and disease management.
