Job summary

Soleo Health Inc. is seeking a Full-time Therapy Care Specialist to support specialty Infusion Pharmacy patients in Norcross, GA and surrounding areas. The role operates under supervision of a Pharmacist to coordinate patient care, dispense logistics, and data capture across the care continuum.

You will coordinate shipments, manage delivery schedules, and ensure accurate documentation. Requires 1+ year as Pharmacy Technician and Alabama license; PTCB/NHA certification preferred.

Qualifications

• Prior experience in home infusion, compounding and/or specialty pharmacy.
• Licensed Pharmacy Technician in Alabama
• High School Diploma or equivalent
• PTCB or NHA Certification preferred
• Prior customer service/patient services experience
• Basic to intermediate proficiency with Excel, Word and Outlook

Responsibilities

• Act as single point of contact for specialty patients and liaison with branch personnel to coordinate medication dispense and delivery.
• Under supervision of specialty pharmacist to capture assessment data, determine supply needs, and schedule new/refill deliveries.
• Run daily and weekly pharmacy reports as needed.
• Answer phones and transfer calls; monitor faxes and refill queues.
• Send orders to physicians for signature.
• Coordinate insurance coverage and authorizations with intake/billing staff.
• Document all patient interactions and care coordination in the system of record.
• Complete therapy-specific training as required.

Soleo Health is seeking a Full time, Therapy Care Specialist to support our specialty Infusion Pharmacy patients in the Norcross, GA and surrounding areas. Join us in Simplifying Complex Care!

The Position:

The Therapy Care Specialist will work with SoleoRx TCMC model and utilize national and local resources to facilitate high quality care of a patient with specialty pharmacy needs. The Therapy Care Specialist will work under the supervision of a Pharmacist to oversee logistics of a patient’s dispense and prescription maintenance, assessment data capture, and serves as the point person to coordinate patient care across all layers of the patient care continuum while on service with Soleo Health. Responsibilities include:

• Act as single point of contact for specialty patients and as liaison with Soleo branch personnel to coordinate timely medication dispense and delivery
• Works under supervision of specialty pharmacist and/or branch pharmacist to capture assessment data, ascertain patients supply needs, and schedule new and refill medication deliveries
• Run daily and weekly pharmacy reports as needed
• Answer phones and warm transfer call to appropriate location, monitor incoming faxes, and monitor TCMC refill queues
• Send orders to physicians for signature
• Work with intake/reimbursement staff to maintain active patient insurance coverage and actively pursue authorizations and re-authorizations with the auth/billing staff at the branch level
• Documentation of all patient interaction and care coordination in the designated system of record for clear and consistent communication
• Complete therapy specific training as required

Schedule:

• Must be able to work 100 percent in office
• Must be able to work Full time, Monday-Friday 8:30am-5pm, 40 hours per week with flexibility as needed
• Must have a minimum of 1 year on the job work experience working as a Pharmacy Technician
• Preferred experience with scheduling refill deliveries for Specialty Infusion Pharmacy patients
• Must be licensed as a Pharmacy Technician in the state of Alabama
• PTCB/NHA Certification preferred

Requirements

• Prior experience in home infusion, compounding and/or specialty pharmacy required
• High School Diploma or equivalent
• Licensed Pharmacy Technician required for the state of Alabama
• PTCB or NHA Certification Preferred
• Prior customer service/patient services experience
• Basic to intermediate efficiency with Microsoft Excel, Word and Outlook – additional programs knowledge of: Insight, CPR+,Genesys, Citus, AlayaCare,
