Job summary

Heartland Alliance is seeking a Therapist/Recovery Counselor to provide individual, family, and group psychotherapy for 18-25 year-olds in supportive housing and through HHCS. Work predominantly in-person across the North Side of Chicago, Edgebrook clinic, and community settings, with some telehealth as needed.

The role emphasizes strengths-based, client-centered, and trauma-informed care, plus community-based case management and collaboration with a client’s care team.

Qualifications

  • Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling.
  • Experience working with children, adolescents, and/or transition age youth and their families.
  • Experience working with people with mental health and/or substance use diagnoses.
  • Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
  • Available to work evenings, based on client need.

Responsibilities

  • Provide strengths-based, client-centered, and trauma-informed clinical services to clients with mental health diagnoses and co-occurring substance abuse disorders
  • Provide comprehensive bio-psychosocial assessment services to clients and their families
  • Develop individualized treatment plans with the input of the client and their team, which includes providers and natural supports.
  • Collaborate with members of the client’s team to ensure quality of treatment and coordination of care
  • Provide both onsite and community-based case management services for clients, as needed
  • Serve as an advocate for clients, linking them to needed community services; assess client’s personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
  • Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Maintain documentation and chart in compliance with Medicaid, CARF, and agency standards
  • Perform other related duties and/or projects as assigned

The Therapist/Recovery Counselor will provide strengths-based, client-centered, and trauma-informed services to clients and their families with mental health diagnoses and co-occurring substance use disorders. They will also provide community-based case management services. This position will mostly be in-person, with services conducted at client homes within the HHCS residential site located on the Northside of Chicago, the outpatient clinic located in Edgebrook in Chicago, & within the community throughout Chicago, as well as some telehealth services, as needed.

Pay Range: $27.88 - 31.25/hr
Schedule: Monday - Friday, 8am - 4:30pm
