Job summary

Palms Behavioral Health in Harlingen, Texas seeks a Therapist PRN to join the multidisciplinary team, delivering psychosocial assessments and a range of therapy services to patients and families.

You will develop treatment plans, coordinate discharge planning, provide crisis intervention, and connect clients with community resources, while maintaining documentation per regulatory standards.

Qualifications

• Master's degree in Social Work, Counseling or Psychology required.
• At least one year of experience as a therapist with all age groups.
• Experience conducting psychosocial assessments and developing treatment plans.
• Ability to provide individual, group and family therapy.
• Discharge planning and care coordination to support transitions.

Responsibilities

• Conduct psychosocial assessment and develop initial discharge plan.
• Develop and implement treatment plans; coordinate any additional services needed.
• Conduct individual, group or family sessions as appropriate.
• Apply psychotherapeutic techniques to treat emotional and behavioral disorders.
• Maintain clinical documentation and progress notes in the medical record.

Therapist PRN | Palms Behavioral Health | Harlingen, TexasAbout the Job:The Therapist I is a part of the multidisciplinary team responsible for providing direct clinical services to patients and their families with most emphasis on completing Psychosocial Assessments; providing individual, group and family therapy for patients; facilitating psycho-educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and providing linkage to outside resources.Roles and Responsibilities:· Conducts a psychosocial assessment and develops an initial discharge plan.· Develops and implements treatment plans for patient based on assessment and coordinates any additional services needed, revising as necessary.· Conducts individual, group or family sessions as appropriate for the treatment plan of the patient.· Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.· Participates in multi-disciplinary treatment planning conferences. Presents appropriate patient information to the treatment team. Recommends effective treatment interventions.· Maintains a caseload as assigned.· Develops and implements special patient groups or program modules.· Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards.· Ensures each assigned patient has a structured and clinically appropriate post hospitalization care plan.
