• Conduct comprehensive behavioral health assessments and screenings.
• Provide brief therapeutic interventions within the Collaborative Care framework.
• Monitor patient progress using standardized tools and symptom tracking.
• Coordinate care among primary care providers, psychiatric consultants, and care teams.
• Manage patients with moderate to high complexity behavioral health needs.
• Ensure timely documentation, regulatory compliance, and quality metric reporting.
• Provide crisis assessment and appropriate intervention when needed.
