Job summary

CAYA Collaborative Care in Kentucky seeks a Therapist II to provide psychosocial assessments and psychotherapy across clinic, community, and home settings. You will collaborate with case managers, psychiatry, and schools, develop person-centered recovery plans, and deliver individual, family, and group therapies using evidence-based practices.

You will participate in intake, crisis response when needed, document in the EHR, and support continuity of care across programs while adhering to state

Qualifications

• Master's degree in social work, counseling, psychology, or related field from an accredited program.
• Must be fully licensed in Kentucky (LCSW, LPCC, LPA, LPP, LMFT or equivalent) and meet QMHP requirements.
• Experience with diverse client populations and evidence-based practices preferred.

Responsibilities

• Conduct psychosocial assessments and diagnostic screenings and develop recovery-oriented treatment plans.
• Deliver individual, family, and group therapy using evidence-based approaches.
• Engage in crisis response and collaboration with interdisciplinary teams.
• Maintain timely documentation in the electronic health record.
• Coordinate with community partners and support services to ensure continuity of care.
