Personalized Health Partners in Akron, OH seeks Licensed Social Workers (LSW) and Licensed Independent Social Workers (LISW) to join as Therapist/Behavioral Health Care Managers. You will provide psychotherapy, group therapy, and collaborative care management to residents in need.

The role offers full-time and part-time options with a $5k sign-on for successful hires, supervision to support licensure, ongoing education, and flexible scheduling to accommodate a patient-focused care team.

Competenze

• Current LSW or LISW licensure in OH.
• Master’s Degree in Social Work preferred but not required.
• Experience with diverse patient populations and neurocognitive disorders.
• Proficiency in using electronic medical records (EMR).

Mansioni

• Deliver individual and group psychotherapy sessions tailored to residents.
• Facilitate collaborative care management with psychiatrists and primary care providers.
• Address mental health needs of residents with chronic illness and disorders.
• Develop and implement evidence-based therapeutic interventions.
• Maintain detailed clinical documentation in an EMR system.
