Job summary

Health Connect America is seeking a licensed therapist to provide high‑quality behavioral health services to children, families, and adults across our network. The role emphasizes trauma‑informed, client‑centered care and collaboration with multidisciplinary teams.

Responsibilities include assessments, individual/group/family therapy, progress monitoring, and community outreach to promote services and support positive outcomes. A Master’s degree and NC licensure are required.

Qualifications

  • Must hold a Master’s degree in Human Services or related field.
  • Must hold one of the North Carolina licenses LPCA, LCSWA, LMFTA, or LCASA.

Responsibilities

  • Conduct biopsychosocial assessments and create individualized treatment plans with clients, families, and treatment teams.
  • Deliver individual, group, and family therapy to address anxiety, depression, trauma, and behavioral disorders.
  • Monitor progress, adjust treatment plans as needed, and provide crisis intervention when necessary.
  • Maintain accurate, timely documentation of all interactions, treatment plans, and progress notes.
  • Collaborate with multidisciplinary teams and participate in treatment meetings for holistic, coordinated care.
  • Connect clients and families with additional resources and support services.
  • Participate in outreach and community engagement to promote services and attract new clients.

Role Overview

As a Therapist at Health Connect America, you’ll provide high‑quality therapeutic services, supporting clients’ mental health and well‑being while collaborating with multidisciplinary teams to ensure comprehensive, client‑centered care.

What You’ll Do
  • Conduct comprehensive biopsychosocial assessments and create individualized treatment plans in collaboration with clients, families, and treatment teams.
  • Deliver therapeutic interventions through individual, group, and family sessions to address mental health challenges such as anxiety, depression, trauma, and behavioral disorders.
  • Monitor client progress, adjusting treatment plans as needed, and provide crisis intervention when necessary.
  • Maintain accurate, timely documentation of all interactions, treatment plans, and progress notes per company and regulatory standards.
  • Collaborate with multidisciplinary teams and participate in treatment meetings to ensure holistic, coordinated care.

  • Participate in outreach and community engagement to promote services and attract new clients.

Through evidence‑based care, timely documentation, and client‑centered collaboration, you’ll help maintain high‑quality mental health services that support positive outcomes for individuals and families.

Qualifications
  • Master’s Degree in Human Services or a related field.
  • Must hold one of the following licenses in the state of North Carolina: LPCA, LCSWA, LMFTA, or LCASA.
