Job summary

St. Vincent de Paul Society of San Francisco seeks a Temporary Case Manager III to provide direct client services at the Division Circle Navigation Center. The role involves case management for about 25–30 clients, developing service plans, and coordinating care with partners.

Position ends approximately 12/1/26 with a 4:00pm–12:30am schedule. The role starts at $30.60/hour, with paid sick leave per SF ordinances.

Qualifications

BA degree in social services, psychology, or related field, or equivalent experience.
Two years’ experience with chronically homeless preferred.
Proficient reading, writing and speaking English.
Experience with Harm Reduction, Motivational Interviewing and OARS.
Knowledge of SF homeless shelter systems and services.
Ability to de-escalate crisis situations and work under pressure.
Strong data entry and record-keeping skills.

Responsibilities

Provide direct client services: counseling, case management, advocacy and accompaniment.
Enter client data into the ONE database within 48 hours.
Create service plans for clients with prioritized issues.
Coordinate care with service providers to meet goals.
Assist clients in maintaining stability and linking to services.
Facilitate meetings and support groups as needed.
Develop client exit surveys and case summaries for outcomes.
Maintain electronic client records, daily logs and statistics.
Participate in staff and case management meetings and trainings.
