Organization Overview Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California's most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. We implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans' services, and health care sectors. The case management department provides intensive case management services to three supportive housing programs in Los Angeles County; Permanent Supportive Housing (PSH) Project- Based, PSH Scattered-Site, and Breaking Barriers. Our programs provide services in different housing setting, including scattered-site and project-based, serving participants that experience homelessness, have a chronic physical or mental illness, people exiting jail/prison or hospital settings. Our team of Case Managers are dedicated to a Housing First approach that values client agency, dignity, and personal growth.
The Temporary Case Manager will provide intensive case management services to a caseload of 20-30 program participants. The Case Manager will assist clients to reach their identified goals and connect clients to community resources and services. The Case Manager will work collaboratively with necessary collaborative partners including the housing department, property management and/or employment teams to ensure that clients reach the ultimate goal of self-sufficiency. The Case Manager plays a critical role in supporting clients to thrive through a client-centered and "whatever it takes" approach to services.
