Job summary

Amity Foundation is seeking an ECM Program Manager to oversee Enhanced Care Management – Community Supports in the assigned county. You will lead day‑to‑day operations, supervise staff, ensure contract compliance, and foster partnerships with healthcare providers and community organizations.

You will recruit and train a multidisciplinary team, analyze data for program decisions, and coordinate resources while maintaining mission alignment and reporting accuracy.

Qualifications

  • Four‑year degree in Social Sciences or related fields and two years of full‑time administrative supervisory experience in a substance abuse program with a justice‑involved population; OR
  • Master’s degree in healthcare administration, public health, social work or related fields and at least two years of administrative supervisory experience managing multidisciplinary teams; OR
  • Six cumulative years of full‑time administrative supervisory experience with a justice‑involved population.
  • Working knowledge of Medi‑Cal and CalAIM initiatives.
  • Ability to read, analyze, interpret, and present data related to the ECM program model and outcomes.
  • Excellent communication and interpersonal skills, with a history of strong leadership and organizational development skills.
  • Ability to relate to diverse populations in a culturally sensitive way.
  • Must use a company vehicle as well as a personal vehicle to visit county jails and transport clients to and from appointments. Must have car insurance, a vehicle, and a valid driver’s license.

Responsibilities

  • Comply with contract requirements, as well as state and federal regulations.

  • Work closely with advocates and other team members to identify necessary clinical and non‑clinical resources to assess enrolled ECM students’ health status and gaps in care.
  • Foster a collaborative culture that encourages teamwork, continuous learning, and excellent student care.
  • Establish and maintain relationships with healthcare providers, community organizations, and stakeholders to enhance service delivery.
  • Provide oversight to billing and claiming processes, ensuring alignment with contractual requirements and MediCal ECM standards.
  • Utilize data to inform program decisions, track performance against goals, and identify areas for improvement.
  • Oversee and coordinate the allocation of designated resources, administer record‑keeping and reporting functions, and ensure compliance with mission, goals, regulations, and guidelines.
  • Evaluate program operations and activities in terms of overall effectiveness and compliance; implement improvements and modifications as necessary.
  • Ensure compliance with Amity policies and procedures and applicable contract obligations.
  • Attend and participate in Amity community building functions.
  • Attend workshops, meetings, and trainings as requested by supervisor.
  • Perform other related duties as assigned.

About the Position

The ECM Program Manager oversees the Enhanced Care Management (ECM) – Community Supports (CS) project in the assigned County. The role supports the Management Team, oversees day‑to‑day administration of programming activities, and provides direct supervision to faculty working on this project.
