Elevance Health is seeking a Telephonic RN Nurse Case Manager I – AmeriBen in Atlanta, GA to assess, develop, implement, coordinate, monitor, and evaluate care plans for members with complex needs.

The role emphasizes care management within licensure, collaboration with Medical Directors, and coordinating resources to improve access and outcomes; full-time, Monday–Friday, 8am–5pm.

Qualifications

• BA/BS in a health-related field is required.
• Minimum of 3 years of clinical experience.
• Current, unrestricted compact RN license in home state required; multi-state licensure if providing services in multiple states.

Responsibilities

• Perform care management within the scope of licensure for members with complex and chronic care needs.
• Conduct assessments to identify individual needs and develop specific care management plans.
• Implement care plans, provide education, and facilitate authorizations/referrals.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans and modify as necessary.
• Interface with Medical Directors and other health professionals on care management plans.
• Assist in problem-solving for access to care, vendors, claims, or service issues.

About This Role

The Telephonic RN Nurse Case Manager I - AmeriBen is responsible for performing care management for members with complex and chronic care needs. This role is ideal for experienced nurses who can assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care.

Highlights

• Location: Atlanta, GA
• Schedule: Full-time, Monday thru Friday 8 am to 5 pm

What You'll Do

• Implement care plans through actions based on assessments, including providing education and facilitating authorizations/referrals.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans and modify as necessary.
• Interface with Medical Directors and other health professionals on care management treatment plans.
• Assist in problem-solving for members and providers related to access to care, vendors, claims, or service issues.

Requirements

Education:

• BA/BS in a health-related field required
