Job summary

Elevance Health is seeking a Telephonic Health Educator - FEP to provide health coaching and disease management support to program members via telephonic sessions. This role emphasizes motivational interviewing, self-management, and collaborative problem solving.

The position is hybrid/virtual with required in-person trainings; candidates must reside within commuting distance of an Elevance Health location or be willing to travel for onboarding and sessions.

Qualifications

  • BA/BS in appropriate field and minimum of 2 years of related experience in direct care health education, exercise instruction, or patient education.
  • Prior health coaching or disease management experience is preferred.
  • Certification or license in the field is preferred.

Responsibilities

  • Provides information to members eligible for disease management programs.
  • Conducts telephonic meetings with program members to provide health information.
  • Diffuses situations with members resistant to health coaching or interventions.
  • Provides consultation to staff on methods to help members overcome barriers to health.
  • Assist in increasing participants' motivation by addressing preventive care and assessing needs.
  • Promotes incremental behavior change leading to healthier outcomes.
  • Conducts behavioral assessments to identify knowledge and needs.
  • May coordinate with the multidisciplinary clinical team.

Telephonic Health Educator - FEP
Location

This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office

Monday – Friday 1:00 pm to 9:30 pm EST

The Health Educator is responsible for delivering health coaching services to disease management program members to support sustainable health behavior change, improve self-management skills, and achieve program outcomes. This role provides education, motivational support, and individualized action planning while collaborating with members to address barriers and promote overall wellness

How you will make an impact
  • Provides information to members eligible for disease management programs.

  • Conducts telephonic meetings with program members to provide health information according to program standards.

  • Diffuses situations with members who are resistant to health coaching and/or disease management interventions.

  • Provides consultation to department associates regarding methods/approaches to help members recognize and overcome barriers to improve their health.

  • Assists with increasing participants motivational level by addressing preventative care, barriers and assessing needs.

  • Focuses on improving member health by promoting incremental behavior change leading to healthier outcomes.

  • Conducts behavioral assessments to identify individual member knowledge skills and behavioral needs.

  • May coordinate with others in the multi-disciplinary clinical team.

Minimum Requirements

Requires BA/BS in appropriate field of specialization and minimum of 2 years of related experience in direct care health education exercise instruction or patient education; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities and Experiences
  • Prior experience in health coaching, disease management in a call center environment, and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.

  • Certification or advanced certification and/or license appropriate to field of specialty preferred.

  • Prior experience utilizing Motivational Interviewing techniques preferred

For candidates working in person or virtually in the below location(s), the salary* range for this specific position is $24.85 to $41.18

Locations: Maine, Nevada, New York; Virginia
