Job summary

Northshore Home Health Care is seeking a Registered Nurse (Cardiac Telemetry) to provide compassionate, evidence-based care to patients with cardiac conditions in a home health setting. You will monitor telemetry, assess changing needs, and deliver individualized nursing interventions.

This role requires an active RN license, strong clinical judgment, and excellent communication to coordinate with the care team and educate patients and families on conditions and next steps.
