Job summary

Navitus Health Solutions is seeking a Telehealth RN Case Manager to deliver Medication Therapy Management (MTM) services for commercial and Medicare members. The role supports clinical outreach programs and collaborates with pharmacy staff to improve member health and wellness.

The position starts January 2027 or later, and requires RN licensure with relevant clinical experience. Join a team focused on high-quality patient care through telehealth and coordinated outreach.

Qualifications

• Registered Nurse with unrestricted licensure.
• Two years of clinical experience preferred.
• Experience in a multi-disciplinary team is preferred.
• Professional PBM industry knowledge is preferred.
• Knowledge of HIPAA and healthcare regulations is preferred.

Responsibilities

• Conduct MTM and expanded clinical programs per CMS guidelines.
• Develop clinical algorithms, pathways, and call scripts for MTM delivery.
• Review algorithms and scripts yearly with updated guidelines.
• Develop documentation standards for all clinical outreaches.
• Assist leadership to develop programs supporting STAR ratings outreach.
• Facilitate communication with clients, providers, and internal staff about MTM outcomes.
• Provide Medicare Part D clinical and educational support and stay updated on regulations.
