Job description

Telehealth Physician - Remote, US

Talent pipeline posting: We are building a pipeline of qualified candidates for future openings in this role. We are not filling this position immediately. We will review applications on a rolling basis and reach out as positions open.

About Carewell

Carewell is a category-defining business that provides trusted caregiving solutions and support for individuals and families. Through Carewell Family Services, we extend our commitment beyond products to person-centered navigation, care coordination, and advocacy services that address social determinants of health needs. Our approach emphasizes compliance, scalability, and high-quality member experiences, while partnering closely with clinicians and community resources to support better outcomes.

About the Role

This is an opportunity to provide clinical leadership for a growing care navigation program at a moment when your judgment will directly shape how it scales. As a Telehealth Physician, you serve as the clinical anchor for Carewell's care navigation and advocacy teams, evaluating members, guiding care plans, and making sure the non-clinical work stays clinically sound.

The role is built around three connected areas:

  • Member Evaluation — conducting telehealth evaluations and identifying the clinical and social needs that drive a member's care plan.
  • Clinical Oversight — reviewing and guiding care plans developed with nurses, social workers, and care navigators.
  • Program Development — building the clinical protocols, escalation pathways, and standards the program runs on.

This program is early-stage, and parts of this role are being built in real time. Processes will evolve and priorities will shift. If that sounds like an opportunity, keep reading.

What You'll Do

Member Evaluation & Care

  • Conduct telehealth evaluations with members who have complex medical, medication, or care coordination needs.
  • Identify and prioritize clinical needs, and translate them into clear next steps for the care team.
  • Coordinate with members' primary care providers and specialists to support continuity of care, rather than replace it.

Clinical Oversight

  • Review and sign off on care plans developed with RNs, LCSWs, and care navigators.
  • Serve as the escalation point for clinical concerns raised by the care team, and respond the same day they are identified.
  • Guide care plans that address both medical needs and social determinants of health, such as food access, transportation, and support at home.

Program & Protocol Development

  • Develop and maintain clinical protocols, escalation criteria, and scope-of-practice guidance for the care team.
  • Support training for nurses and care navigators on clinical red flags, medication basics, and when to engage.
  • Partner with Compliance and program leadership on clinical quality, documentation standards, and new program design.

Documentation & Compliance

  • Complete accurate, timely documentation for every evaluation and care plan review.
  • Maintain licensure and credentialing, and follow all telehealth, privacy (HIPAA), and consent requirements.

KPIs You'll Drive

Clinical Quality

  • Care plan review and sign-off completed on schedule
  • Clinical escalations responded to the same day they are identified
  • Quality outcomes for engaged members, such as avoidable ED visits and readmissions

Program

  • Clinical protocols and escalation pathways built, maintained, and adopted by the care team
  • Documentation accuracy and timeliness

Who You Are

Required

  • Availability to work Monday–Friday, 9am–6pm ET.
  • Located in the United States and authorized to work in the U.S. without employer sponsorship.
  • MD or DO with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active, unrestricted medical license in at least one U.S. state, and willingness to obtain additional state licenses as the program expands.
  • 3+ years of post-residency clinical practice.
  • Experience working in a team-based care model with nurses, social workers, or care coordinators.
  • Strong understanding of social determinants of health and how they affect outcomes.
  • Comfortable with telehealth platforms, EHRs, and care management tools.
  • Comfortable with ambiguity and rapid iteration. You can help build a program while it runs.

Nice to Have

  • Interstate Medical Licensure Compact (IMLC) eligibility or multiple active state licenses.
  • Active DEA registration.
  • Experience with Medicare, dual-eligible, or older adult populations.
  • Transitional care, value-based care, or population health experience.
  • Experience in a startup or new program launch.
  • Bilingual (Spanish preferred).

Why This Role

  • Ground-floor clinical leadership. You will shape how the program practices and scales.
  • Medicine focused on the whole member, with a team that handles coordination and follow-through.
  • Meaningful, mission-driven work with visible impact on members' lives.
  • Close partnership with care navigation leadership and program operations.
