Job summary

Belle is hiring a Board Certified/Eligible Primary Care Physician for remote, full-time telehealth to serve Medicare beneficiaries. The role focuses on preventive care and chronic condition management, with multi-state licensure preferred. Work from home Monday–Friday 9am–6pm, no on-call requirements.

Qualifications

• MD/DO medical degree
• Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license with multi-state licensure a plus
• Strong interest in preventive care and senior health
• Tech-savvy with telehealth experience
• Excellent communication and collaborative mindset

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with integrated care team (nurse case managers, NPs, CHWs)
• Ensure thorough documentation in EHR and compliance with quality measures
• Engage patients in preventive care to reduce avoidable complications

What We’re Looking For

• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative, mission-driven mindset with excellent communication skills
