Job summary

Integrated Health 21 is seeking compassionate Nurse Practitioners for a flexible PRN telehealth role based in Pittsburgh, PA. This position offers an average of 10–20 hours per week and requires obtaining licensure in West Virginia to support our growing wellness programs across multiple states.

In this role you will conduct individual virtual wellness evaluations, educate participants on prevention for diabetes, heart disease and stroke, record visits in an EMR, and tailor action plans.

Qualifications

• Master of Science in Nursing required.
• Family Nurse Practitioner credential required.
• Doctor of Nursing Practice degree preferred.
• Current WV license.
• CPR certification.
• At least one year of NP experience in primary care or internal medicine.
• Strong computer and telehealth skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct virtual wellness evaluations using telehealth software.
• Educate participants on health risk status and refer to eligible employer programs.
• Record, host, and document consultations in EMR.
• Develop individualized action plans based on results.
• Assess depression risk and refer to resources as appropriate.
• Refer participants to employee carrier resources based on results.
• Engage participants in creating a future health plan.
• Oversee lab results and communicate any critical findings.
