Job summary

Ayble Health is seeking a part‑time Nurse Practitioner to deliver evidence‑based, whole‑person care for chronic GI conditions via telehealth. You will be a core member of a multidisciplinary team, partnering with physicians, dietitians, pharmacists, and behavioral health specialists to support patients longitudinally and implement care plans.

The role emphasizes patient relationships, behavior change, and ongoing care between visits, with access to training and resources within Ayble's

Qualifications

• MSN, Post‑Master's Certificate, or DNP from an accredited NP program.
• Current national NP certification (ANCC/AANPCB or equivalent).
• Active RN and APRN licenses in at least five states.
• Commitment to a minimum of 30 hours per week.
• Experience working in multidisciplinary care teams.
• Experience supporting chronic GI disease management and longitudinal care.
• Familiarity with Motivational Interviewing or similar behavior-change approaches.

Responsibilities

• Collaborate with a multidisciplinary team to deliver coordinated, whole-person care.
• Develop individualized care plans including nutrition, behavior health, and meds.
• Evaluate symptoms, order labs when needed, and escalate with supervising physicians.
• Provide longitudinal telehealth and asynchronous support to patients.
• Use evidence-based communication to promote engagement and self-management.
• Adhere to clinical guidelines (AGA/ACG) and ensure documentation quality.
• Coordinate with patient’s existing providers and facilitate referrals.
• Deliver care through Ayble’s telehealth platform with high clinical standards.
• Contribute to continuous improvement of workflows, education materials, and protocols.
