Qualifications

• Active RN license in good standing.
• Compact licensure enabling multi-state practice.
• Telehealth or care management certifications preferred.

Responsibilities

• Conduct telehealth outreach, provide education and care coordination.
• Monitor patient progress and update the clinical team.
• Support patient engagement and adherence to care plans.
• Collaborate with multidisciplinary teams to meet goals.
• Participate in pilot workflows and offer process feedback.
• Document patient interactions in EHR and designated platforms.
• Support Chronic Care Management activities when applicable.
• Maintain compliance with healthcare regulations and privacy rules.
• Exercise professional judgment within licensure to deliver patient care.
• Maintain licensure and multi-state eligibility across jurisdictions.
