Responsibilities

• Provide urgent health care via telephone and tele-video to Home and Community-Based Services patients.
• Develop and implement clinical plans of care for adults with chronic/complex conditions.
• Gather history, perform exams as needed, and create treatment plans.
• Prescribe medications as allowed by state authority.
• Maintain patient medical records and coordinate with the multidisciplinary team.
