Qualifications

• MSN, Post-Master's Certificate, or DNP from an accredited NP program.
• Current national board certification as NP (ANCC/AANPCB or equivalent).
• Active, unrestricted RN and APRN licenses in at least five U.S. states.
• Ability to commit to a minimum of 30 hours per week.
• Experience collaborating within a multidisciplinary care team.

Responsibilities

• Collaborate with a multidisciplinary team to deliver coordinated, whole-person care.
• Develop individualized care plans incorporating nutrition, behavioral health, and lifestyle changes.
• Evaluate patient symptoms and interpret labs; escalate to supervising physicians as needed.
• Provide longitudinal telehealth support and asynchronous communication to implement care plans.
• Promote patient engagement using evidence-based communication and behavior-change strategies.
• Adhere to clinical guidelines from AGA and ACG.
• Coordinate care with patients' existing providers and referrals.
• Deliver care virtually via Ayble's telehealth platform with high quality and thorough documentation.
• Contribute to ongoing development of clinical workflows and patient education resources.
