As a Team Ldr-CCS RN, you will coordinate clinical workflow and oversee the daily activities of a Community Care Services nursing team, such as Hospice or Home Health Care. You’ll ensure safe, effective, and compliant patient care while serving as a clinical resource and liaison between leadership and staff. Your role is vital to maintaining high standards of care and supporting both your team and the patients we serve.
