Team Lead, Care Coordination Specialist
Job Description

Purpose & Scope:

Supports patients across the continuum of care by coordinating transitions of care, conducting patient outreach, supporting quality and care gap closure, and facilitating effective care coordination within the ambulatory setting between patients, providers, and post-acute settings. Functions within a multidisciplinary care team to improve patient experience, clinical outcomes, and performance on quality and population health initiatives.

Provides day-to-day functional leadership for Patient Care Representative (PCR)-level and Medical Assistant (MA) Care Coordination Specialists while maintaining an active care coordination workload. Supports transitions of care, outreach, and quality initiatives, and serves as a liaison between frontline staff and leadership.

Education:

High school diploma or equivalent required.

Experience:

  • Three years of healthcare/medical (hospital or clinic/office) experience with exposure to medical terminology is required.
  • Prior experience in a Team Lead or equivalent capacity within a healthcare setting, with demonstrated ability to coordinate team workflows, prioritize assignments, provide peer mentoring, and serve as the first point of escalation for operational or care coordination issues strongly preferred.
  • Prior experience in transitions of care, care coordination, outreach, ambulatory care, population health, and/or quality program support strongly preferred.
  • Experience supporting frontline staff (e.g., PCRs, Medical Assistants, or care coordination teams) while maintaining an active individual workload strongly preferred.
