Job summary

Responsibilities

• Gathers pertinent information from insurance carriers, financial counselors and ancillary staff to ensure patient benefit coverage for post-discharge services.
• Confers with the TCM team to prioritize placement requests and partners to provide post-acute services.
• Performs active listening and guides discharge care goal communication for patients and families; provides educational materials and answers questions related to automated symptom checker, video and texting with TCM team.
• Performs other duties as assigned.

Responsibilities

JOB SUMMARY

Assists in the administration of Transitional Case Management (TCM) and discharge Planning functions to include, but not limited to, data entry, organization of workflow, communications, critical information tracking, report creation, and links with community resources. The position is focused on supporting the role of the Transitional Case Management (TCM) department in optimizing the Transitional Care Management and Discharge Planning process, efficiency and effectiveness.

Essential Functions

• Performs a variety of routine clerical activities. Assists TCM team in assigned area of responsibility, including:
• Compiling information, organizing paperwork and files, providing information, coordinating with outside agencies, monitoring the status of cases, and clinical data entry. Research, compile, review, reconcile, and/or maintains a variety of information and records. Answer questions and supplies detailed information both in person and on the phone to patients, case managers, and other service agencies. Receives and processes a variety of requests for information, referrals, forms, orders, notices, and/or other related information. Follows through on patient needs through coordination of services.
• Performs quality monitoring and data collection activities. Is trained and performs high quality customer service standards. Developed strong collaborative relationships with TCMs and acute/PA acute heath care partners.
• Distributes post-acute placement requests to preferred provider network resources and identifies availability. Maintains routine communication with referring TCM team member of the status of requests. Obtains forms to initiate and complete placement process. Arranges safe transportation which fits the patients’ needs. Gathers pertinent information from insurance carriers, financial counselors and ancillary staff to ensure patient benefit coverage for post-discharge services.
• Confers with the TCM team to prioritize placement requests. Partner with the TCM team to provide post-acute services. Facilitate prompt processing of all requests for post-acute services, equipment, or appointments with community resources.
• Performs active listening, customer service skills, and guides discharge care goal communication for the patient and families.
• Provides patient educational materials and answers questions related to automated symptom checker, video and texting communication with TCM team members.
• Performs other duties as assigned.

EDUCATION

• High School Diploma or GED Required
• Associates or health care related advanced education. Preferred

Experience

• 2 years in healthcare related position Required
• 4 years experience in a healthcare related field Preferred

LICENSE AND CERTIFICATIONS

• Basic Life Support (BLS) - Certification Required
• Health Care or Business-Related Certification Preferred
